Comment on the readability of this article.

Very readable, but laced with so much medical terminology, the lay person would have a difficult time understanding all the terminology.

Course Topics Covered:

1.      Paraquat poisoning

2.      case study of patient with paraquat poisoning

3.      manifestations of paraquat poisoning

4.      initial management of paraquat poisoning

5.      Prognosis of paraquat poisoning

Brief Summary:

This article concerns the case study of a patient in Thailand who attempted suicide by ingesting paraquat  At first there seemed to be no serious medical problems, but the electrocardiogram and the urine dithionite test suggested otherwise.  She was admitted to intensive care and given fluids and 100 g of activated charcoal plus 100 mL of 70% sorbitol via a nsogastric tube. By day 2 her pain was

increasing and all blood chemistries were elevated.  By day 3  the chest Xray revealed bilateral infiltrates, pneumomediastinum, and pneumopericardium.  Her renal function deteriorated.  Progressive multiple-organ failure ensued and she died on day 4.  The article described the progression of this poisoning and attempts at each stage to alleviate suffering.  There is no specific antidote.  Patients have a poor prognosis.

Identify and attempt to explain three (3) terms, procedures or concepts that were unfamiliar to you when reviewing this article.

1.      Poisoning by pesticides and other agricultural chemicals is a major public health problem worldwide, especially in developing countries, causing about 20,000 annual fatalities.

2.      Urine dithionite test (confirms the presence of paraquat).

3.      The clinical manifestations of paraquat poisoning range from local irritation to multiple-organ failure and death.

Detail the strengths/weaknesses of this article.

This article is very strong in explaining the testing for paraquat poisoning and detailing how the poison affects the organs.

The only weakness that I can see is that it uses so many technical/clinical term (which may be necessary given the seriousness and the viewpoint of the author).

How might this article be useful to you in your further studies and career?

This article would be useful in diagnosing someone in the ER who had exposure to paraquat poisoning with procedural guidelines on how to treat them and what to expect as lungs are effected and pulmonary failure occurs.

Does the article include a list of references/citations that can be used to find more information on the topics they cover?

Yes, it has a very detailed reference list.
